If not what is the difference? My Father-in-law appointed me the advance directive and durable power of atty. on one form.

Setting up POA -- durable or springing -- and healthcare proxy are often done as part of the advance directives. The advance directives also have instructions on what to do at the end of life. It addresses issues like extraordinary measures, feeding tubes, and resuscitation if the heart stops beating.

On this form he has circled unless he becomes a burden to the Family. Is living alone and 24 hour supervision is recommended by health officials because he a fall risk being a burden. He falls continuously and bleeds uncontrollably for days from the contusions he suffers. He refuses medical treatment and will not let the neighbors call us. He forgets to turn water faucets off. We are always waiting for a phone call and fear what it will be.

A DPOA Is a Durable power of Attorney for health care and finances. Advanced Directive ask you on the form if you want everything be done possible to save your life even if it means being put on a respirator or breathing machine or do you just want to be taken off meds and just comfort measures taken until you pass

You may want to call adult protective services to do a check on him. If he's falling that often that he's constantly unsafe then they will be able to help.
