My mom currently lives with me in Texas. She has Alzheimer's and needs long-term facility care. She wants to return to North Dakota. If we plan to move her into a facility in ND, do I apply for Medicaid in TX or ND

She is only eligible for Medicaid in the state which she resides. You can not apply for Medicaid in ND until she is actually there.

ND is approximately 20 hrs or more away. Over 1200 and that depends on what part of the state your leaving from and going to.

Yes, Medicaid differs by State. In my State each county has their own office. You apply in the County the facility is in.

If you are her only living relative and she moves to ND, you might have to do several trips there. What if she gets hospitalized and you need to deal with legal issues. It might be best for her to live as close as possible to family. Don't count on her old friends to visit

Establish North Dakota residency (for the app you will need to supply the ND facility address, intent to remain, transfer of records from TX, etc.)

Medicaid is a state run program. You apply in the state that you will be placing her into a facility for care. You might consider finding an attorney familiar with ND Medicaid rules since each state is run differently. She will need to prove residency. There may not be an available bed where you and she want to go. If there is no one local to visit and manage care, don’t count on friends or family for visits. And moving her to another state with all new doctors will be a challenge too.
