Maureen Jensen is owner/managing member of Jensen Case Management, LLC which was established in 2014 with a mission to provide all clients access to quality and customized case management services. Areas of expertise include disability management, chronic disease care management, and consultations. Jensen Case Management is introducing Aging Life Care ™ services for older adults and persons with disabilities. Utilizing a unique team and network of Aging Life Care ™ professionals, Jensen seeks to deliver comprehensive and wholistic care coordination to seniors, persons with disabilities. Aging Life Care ™ professionals can fill a niche for those seniors “elder orphans”, living far from family and desire to maintain highest level of independence. It may also benefit those seniors and their families who need assistance with navigating and accessing existing community services.
The cornerstone of Aging Life Care ™ is based on using 8 Core Knowledge Areas that are used to develop individualized, client centered, plans. These services include, but not limited to: initial interviews and home assessments, comprehensive & individualized care plans, identification of daily challenges, suggestions for solutions, & referrals to providers and community resources. Housing transitions and suggestions for maximizing resources & safety are also addressed. Aging Life Care ™ professional are also able to liaison between seniors, their families and their providers of care. They offer education, support and advocacy.
MAUREEN JENSEN, RN, BSN, CCM, MPA
Maureen Jensen brings a unique blend of education and experience as a community health educator and registered nurse of 30 years, to the field of case management and Aging Life Care ™. She is certified case manager (CCM) since 1995, earned master’s degree in Public Administration (2009). Maureen is a member of the Case Management Society of America (CMSA- since 1995) and is an Advanced Professional Member of the Aging Life Care Association. Maureen has been a Lions Club member for 15 years and through the Marquette Lions Club has provided for the past 3 years, a scholarship for graduating high school seniors going on to study nursing in college. As a creative problem solver, effective communicator and motivator, Maureen has over 30 years of experience as registered nurse, coordinator and liaison with providers, families, and clients with catastrophic/complex, challenging conditions in a variety of settings. Maureen’s life philosophy includes “giving back”, being focused and action driven, and always moving forward. Assisting others to overcome obstacles and reach meaningful goals, is the cornerstone of her practice.
