We are an affordable alternative to assisted living facilities and nursing homes – we help our clients to stay in the comfort of their own homes. With our in-home care services, clients will not need to rely on family members, friends and neighbors.

We also provide home care services to clients that are residing in a hospital, assisted living or skilled nursing facilities that may need extra personal attention. These home care services range from companion care and personal care to 24/7 and Live-In care, skilled nursing services, and more.

Our History

We know how overwhelming the responsibility of in-home care can be. Having received a call for help from an elderly and disabled neighbor back in 2003, we did not hesitate to help her while her daughter was away. This was a turning point in our lives – we knew exactly what we wanted to do.

In 2004 we opened the doors to our home care business so that we could provide the full range of in-home care services to individuals and families. We also discovered that providing in-home care for a family member is not easy, nor is it something most family members are prepared to do. The burdens of family caregiving can lead to resentment, burnout, and friction among family members.

Before in-home care starts, our no-obligation, face-to-face in-home meeting gives opportunity to clients and family members to ask questions and learn more about our personalized in-home care services tailored to suit their individual needs. As a result of this meeting, a caregiver is assigned that has the necessary skills, experience and personality match to best serve the client’s needs.

Heritage Home Care's Registered Nurses identify specific needs of each client and design a customized Plan of Care to enhance quality of life for the client and family. Our Nurses perform initial assessments and make periodic visits to monitor progress and client satisfaction. Our caregivers are closely supervised to ensure that the client is receiving the best possible care and services.

Credentialing and Training

All prospective nurses and caregivers undergo an intensive pre-employment screening process, which includes a criminal background investigation, verification of references, and employment history. The employees are also subject to all state and federal health requirements – they must be free of communicable diseases and must be physically able to fulfill their in-home job requirements.

All nurses and caregivers are bonded and insured, experienced, skills tested, and undergo continuing education and training in senior home care. The caregivers are closely supervised, monitored and managed by Heritage Home Care supervisors and Registered Nurses.

All caregivers are periodically evaluated using performance assessments.
