Gateway to Care Management provides comprehensive care management and healthcare navigation services designed to support older adults and individuals with chronic conditions. Services focus on improving safety, clarity, and quality of life through structured assessment, coordinated planning, and ongoing monitoring across medical, functional, and social needs.
Core support includes care management, geriatric assessments, and aging-in-place planning designed to help families make informed decisions during complex care situations.
The organization specializes in care coordination across the full healthcare continuum, including hospital systems, rehabilitation settings, home health services, and long-term care environments. Additional services include dementia and Alzheimer’s support, fall prevention strategies, crisis intervention during medical events, and relocation assistance when transitions in living arrangements are required.
