We set up power of attorney when my mother was able to make decisions and sign the document herself. Now we want to add my brother as a second person on the power of attorney document but my mother is no longer able to make decisions or even sign her name. How can we add him?

You can't add him, unfortunately. Is this POA for health care or finances?
The courts would need to get involved if you were unable to fulfill your duties as her primary agent.

You can't because Mom has to assign another POA.

My mom set hers up as me as her POA then my brother if I am unable. I think most lawyers will recommend naming a second person if the first person named is no longer able (say I were to predecease her).

What are you wanting your brother to do? He doesn't necessarily need POA to help out.

You cannot. Legal documents can only be drafted and signed by a person when they have a sound mind to do so.
